Client Agreement

Thank you for choosing NECTAR body•face•skin. We are committed to providing each guest with an exceptional, personalized experience in a boutique luxury setting. This Client Agreement outlines the terms and expectations that govern your relationship with NECTAR. By booking an appointment or purchasing services or products, you acknowledge and agree to the terms set forth in this Agreement.
This Agreement is intended to complement — not replace — the health intake forms, informed consent documents, and clinical disclosures you will complete through our secure patient portal (powered by Zenoti) prior to receiving services. Those clinical documents address treatment-specific risks, contraindications, and health history and remain binding independent of this Agreement.
1. Scheduling and Appointments
Booking
Appointments may be booked online through our patient portal, by phone at (517) 306-2510, or in person at our DeWitt location. A valid credit card is required at the time of booking to secure your reservation.
Cancellation Policy
We kindly ask that you provide at least 48 hours' notice before cancelling or rescheduling an appointment. Cancellations or reschedules made within 48 hours of the scheduled appointment time will result in a $100 non-refundable cancellation fee charged to the card on file.
No-Show Policy
Failure to arrive for a scheduled appointment without prior notice ("no-show") will result in a $100 non-refundable fee charged to the card on file. Guests who repeatedly fail to honor scheduled appointments may be discharged from the practice at NECTAR's discretion.
Arrival
Please arrive at least 15 minutes prior to your scheduled appointment to allow time to check in, complete any required forms, and settle into our space. To preserve the tranquility of the environment, we ask that you silence all electronic devices upon arrival.
Late arrivals may result in reduced treatment time to remain on schedule for subsequent guests, or in the appointment being rescheduled entirely. The full service fee or cancellation fee may still apply at NECTAR's discretion.
2. Financial Terms
Payment
Payment is due in full at the time services are rendered. NECTAR accepts major credit cards, debit cards, and gift cards. Cash is not accepted. Pricing for services and products is subject to change without prior notice.
Gratuity
Gratuities are appreciated but never expected. Tips may be added at checkout and are distributed directly to your service provider.
Refund Policy
Due to the nature of aesthetic services, all sales are final. NECTAR does not offer refunds for completed services. If you are dissatisfied with your results, please contact us within 14 days of your appointment so we may discuss options, which may include a complimentary follow-up or adjustment at NECTAR's discretion.
Retail product returns are accepted within 14 days of purchase in unopened, original condition with proof of purchase. Opened products cannot be returned for hygienic and safety reasons.
Packages and Memberships
Prepaid treatment packages and memberships are non-transferable and non-refundable unless otherwise stated in writing at the time of purchase. Package terms, including expiration dates and usage conditions, are provided at the point of sale.
3. Gift Cards
NECTAR gift cards are available for purchase online and in-clinic. The following terms apply:
- — Gift cards are not returnable or refundable for cash, except where required by applicable law.
- — Gift cards may not be resold or used in connection with any marketing, advertising, or promotional activities.
- — Lost or stolen gift cards will not be replaced. NECTAR is not liable for unauthorized use of gift cards.
- — Gift card balances do not expire unless otherwise required by law.
- — Use of NECTAR's name, logo, or trademarks in connection with the purchase or distribution of gift cards is prohibited without prior written consent.
4. Health, Safety, and Consent
Your safety is our highest priority. Prior to receiving any clinical or aesthetic services, you will be asked to complete health intake and informed consent forms through our secure patient portal. These forms address your medical history, current medications, allergies, contraindications, and treatment-specific risks. It is your responsibility to disclose all relevant health information accurately and completely.
If you have existing medical conditions, you are advised to consult your physician prior to booking services. Please inform your provider of any changes to your health status at each visit.
Illness
If you are experiencing symptoms of illness, we ask that you reschedule your appointment and consult your primary care physician. Our priority is the safety and well-being of all guests and team members. Additionally, we recommend avoiding filler treatments two weeks before and after any vaccinations.
Right to Refuse or Discontinue Service
NECTAR reserves the right to refuse or discontinue any service at any time if, in the professional judgment of the provider, proceeding would pose a risk to the health or safety of the guest, provider, or other guests.
Minimum Age
Guests under the age of 18 must have a consent form signed by a parent or legal guardian. NECTAR reserves the right to decline service to any minor, even with guardian consent, at its sole discretion.
5. Privacy and Data
NECTAR collects and maintains personal information necessary to provide aesthetic and clinical services, including your name, contact information, health history, payment information, and treatment records. This information is managed through our secure practice management system (Zenoti) and is handled in accordance with applicable privacy laws and regulations.
How We Use Your Information
- — To deliver, coordinate, and follow up on your treatments and care plan.
- — To process payments and manage your account.
- — To communicate appointment reminders, follow-up instructions, and practice updates.
- — To comply with legal, regulatory, and professional licensing requirements.
Information Sharing
NECTAR does not sell your personal information. We may share limited information with third-party service providers (such as Zenoti for scheduling and records, and payment processors) solely to operate our practice. We may also disclose information where required by law or to protect the safety of our guests and team.
Your Rights
You may request access to your personal information, request corrections, or ask questions about our data practices by contacting us at info@nectarbodyfaceskin.com. For treatment records, requests will be processed in accordance with applicable state and federal regulations.
Communications
By providing your contact information, you consent to receive appointment confirmations, reminders, and follow-up communications via text, email, or phone. You may opt out of marketing communications at any time by contacting our office or using the unsubscribe link in any marketing email. Opting out of marketing does not affect appointment-related communications.
6. Photography and Social Media
With your separate written consent, NECTAR may photograph treatment areas before and after services for your personal treatment record. These images are stored securely in your patient file and are not shared externally without your explicit permission.
If you choose to allow NECTAR to use your images for educational, marketing, or social media purposes, a separate Photo Release and Consent Form will be provided. You may revoke this consent at any time by notifying us in writing. Revoking consent does not affect prior authorized uses.
7. Limitation of Liability
NECTAR body•face•skin, its owners, providers, and staff shall not be held liable for any adverse reactions, complications, or dissatisfaction arising from services rendered, provided that treatment was performed in accordance with accepted professional standards and that the guest disclosed all relevant health information as required.
By receiving services at NECTAR, you acknowledge that aesthetic and clinical procedures carry inherent risks, which are described in detail in the informed consent forms completed prior to treatment. You agree to follow all pre- and post-treatment instructions provided by your provider.
8. Changes to This Agreement
NECTAR reserves the right to update or modify this Client Agreement at any time. The current version will be available at our office and on our website at www.nectarbodyfaceskin.com. Continued use of NECTAR's services after changes are posted constitutes acceptance of the updated terms.
